The Medical Home is an innovative, team-based approach to providing health care services.  A partnership develops between the patient, his or her primary clinician, and a health care team.  Together, following evidence-based guidelines for medical care, the team will coordinate to provide the best health care services possible for you.

  • Our goal is to be your partner in healthcare by serving as your medical home.  We are committed to make available to you a personal physician who provides for all of your healthcare needs and coordinates your care across all settings, including the medical office, hospital, clinics, testing facilities, and other places where you receive your healthcare.

  • You may request care and advice by calling 816-781-7400 during our office hours. If you need care or advice after office hours, please call 816-781-7400 and the answering service will direct your call to the provider on call. Our office hours are Monday through Friday: 8:00 AM - 5:00 PM and the 1st and 3rd Saturday of each month from 8:00 AM - 1 PM.

  • We expect you to provide us with your medical history, as well as information about any care you obtain outside of our practice, to include your current medications, medical history, health status, recent test results, self-care information, and data from recent hospitalizations, specialty care or ER visits.

What you can expect:

Team-Based Care:

  • One of us will remain as your Primary Care clinician.  However, we will have a team of nurses and support staff to work with you to meet all of your health care needs.  

Improved Health Access and Communication:

  • For urgent care issues during working hours, your Primary Care clinician (or another one of our team members) will attempt to see you on the very day that you have an urgent health care need.  You will simply need to call the main office number during working hours to schedule a same-day appointment with us.  Many urgent health care needs can be handled by your Medical Home team.  You will then avoid having a prolonged and expensive visit to the Emergency Room.

  • Better communication and cooperation between patients and their care team while assuring optimal individualized medical treatment.
